New Patients
In order to fill your prescription, we require certain information
from you. This is a short 2-step process:
Step 1
1. Fill out the Patient Questionnaire and Release Form
You will be required to complete a patient questionnaire and to
sign a release form. The patient questionnaire and prescription
are reviewed by a licensed Canadian physician. This enables us to
safely and efficiently fill your prescription. Any information you
provide will be kept strictly confidential.

Step 2
2. Send us the information.
Simply mail the forms back to us, or fax them back to us toll-free
at 1-866-783-4223. Please remember to include your valid
prescription with the questionnaire and release form. Make sure the Order Code "JMA"
is on the form to receive the latest prices.

Approval and Shipping
We will contact you to confirm all orders that we receive by fax or
mail. Please allow up to 21 days after receiving a confirmation from our
representatives for your medication to be delivered to your home.
Our goal is to get your order processed, shipped and in your hands
within 21 days. Occasionally, there may be some delays that are beyond
our control.
At this time, government regulations limit the quantity of medication
we send you to a 3 months supply. If your prescription allows refills,
you can order these refills from us every 3 months. You are not required
to fill out another questionnaire or release form to order a refill.
Once the refills are exhausted, you will be required to submit a new
prescription.
We DO NOT ship controlled substances such as
amphetamines, benzodiazepines (e.g. Valium), or narcotics such as
codeine and morphine.
Under pharmaceutical law, prescription drugs are not returnable.
All sales are final.
